Housing Stability for Youth in Courts (H-SYNC) Feedback Survey
INFORMATION & Consent Page
Welcome to the feedback survey for Kitsap County's Housing Stability for Youth in Courts (H-SYNC) referral program, which juvenile court staff were asked to take part in over the last few years and involved court staff referring youth and families at risk of homelessness or experiencing home conflict to a Family Navigator. In Kitsap County, those Family Navigators were Cassandra Elowski and Denise Williams at Kitsap Mental Health Services (KMHS). The purpose of this survey is to collect court staff feedback on the H-SYNC program itself. This feedback will help us understand how staff perceive the program and identify areas for improvement for future youth, families, and probation staff.
The survey takes about 20 minutes to complete. Responses will be held on the secure Qualtrics site and then transferred and stored on a secure server at the University of Washington. Responses will be presented anonymously/in aggregate form.
The benefits of participation include helping to discover and improve new programs for court-involved youth as well as donating $20 to one of several non-profits of your choice (chosen at the end of the survey).
This study is funded by the Raikes and Block-Leavitt Foundations and is carried out by the University of Washington's CoLab for Community & Behavioral Health Policy. For more information about the study, contact hsync@uw.edu.
Continuing with this survey indicates your consent to participate.
Thank you!
